I can independently execute a Root Cause Analysis (RCA) or act as an Expert Witness on a problem that has happened in your Software Development Manager or Software Development Lifecycle (SDLC).
The Root Cause Analysis can be accomplished remotely, on-site, or a combination of the two. Most of the RCA will require statements and documents from the team from which we can further investigate using email, face-to-face, and telephone video conferences as necessary.
Benefits of an Independent Root Cause Analysis
A root cause analysis is often the beginning of a change to a culture or an environment. These are often met with resistance that permanent employee’s do not feel comfortable presenting, or feel pressured to lean one way or the other for fear of offending someone. An independent has no “internal politics” to deal with and is free to gather the findings without prejudice.
Objective
The objective is to work with your team to:
- Investigate and identify the root cause(s) of a problem
- Establish a sequence of events that led to the problem
- Determine what preventative action(s) if any is required to prevent reoccurrence.
Output
A document formatted to your specification will be produced.
